A jury inquest has concluded that a hospital failed to properly risk-assess a room where a man hanged himself in March 2017.
Ryan Whiteman, 24, was found dead in his room at Fieldhead Hospital, Wakefield where he had been detained under the Mental Health Act since 2014.
Mr Whiteman had told staff he had suicidal thoughts and was under 30 minute observations at the time of his death. The hospital had failed to correctly asses the ligature points in the room and in particular the bathroom door.
